The Care Quality Commission found faults in almost every area.
Inspectors from the Care Quality Commission (CQC) last visited the site almost two years ago, when it was also labbeles as 'requires improvement.'
They said they needed to see changes to the way services are run, prior to this latest inspection.
Their are five key areas that are investigated.
They are, safety, effectiveness, responsiveness, levels of caring, and leadership.
In the report inspectors said:
"We rated safe, effective and responsive as requires improvement, and caring as good.
We rated six services as requires improvement, one as inadequate, nine of the trust’s services as good and one service as outstanding.
We rated caring for community services as outstanding.
We rated well-led for the trust overall as inadequate."
